Transcript & Records Request
Please have the following ready:
- Full name used at school
- Date of birth
- The year you graduated or left school
- If you would like records or transcripts to be mailed, please provide me with a complete address where to send the transcripts
Note:
- If you left or graduated before 2008, an index number will be provided to you.
- Once you have already received your index number, you must request transcripts/records with the LAUSD Records Center at https://lausd.edutranscend.com/
