All athletes will now get cleared through www.athleticclearance.com
Click on "CA"
Create an Account
Start a new clearance - Make sure to select 'MONROE" and the correct year.
Then complete ALL sections.
STEP 2
You MUST upload your COMPLETED ( BOTH FRONT AND BACK) , physical form and a copy of the athletes medical insurance card into athleticclearance.com
Preparticipation Form Checklist:
- Fill out the entire form completely AND explain all the yes answers in the space provided or add additional paper if needed.
- Make sure it is signed by both the parent/guardian and student-athlete.Physical Form Checklist:
- Make sure the entire form is filled out and the doctor signs AND stamps the physical
Physicals are good for 1 year from the date of the physical
